Credentials tracked in a workbook that was accurate in March, maintained by whoever had time, and never quite reconciled against who's actually on the field.
An expiry date that passed in May. Nobody was watching for it, because watching for it is somebody's unpaid Tuesday night.

Most systems record a credential and trust someone to act on it. GovSport treats an expired credential as a change in access, not a line in a report.
